Do these need a garter belt?

No. The suspender look comes from the built-in side and back cutouts, so there's nothing to clip or adjust. Pull them on like regular pantyhose and you're set.

Will one size actually fit me?

They stretch to cover most frames from about a US 4 to 14 and a 5'2" to 5'9" height. If you're taller or fuller in the hip, the sheer knit will sit a little lower and thinner, which is normal for one-size hosiery.

How sheer are they, really?

Very. This is a see-through nylon weave, not an opaque tight, so your skin shows through everywhere except the reinforced areas. Great for layering or the bedroom, not for full coverage.
