Does the top actually leave the bust open?

Yes. The cups are cut away, so your breasts stay fully exposed while the frame and straps stay on.

Will one size fit me?

It's cut to fit most bodies from 5'0" to 5'10" and 90 to 175 lbs. The fishnet has real stretch, but if you're near the top of that range expect a snugger fit.
